Form SH-4
What it is
SH-4 is the instrument of transfer used to move shares from one holder to another — a sale or a gift, where both sides are alive and agreeing.
It is frequently confused with transmission, which is what happens when a shareholder dies. Transmission does not use SH-4, and using the wrong one is a common reason a submission comes back.
When you need it
Shares are being transferred between two living parties.
You are gifting a holding to a family member.
A transfer was agreed but never completed and the paperwork has to be redone.
When you don’t
The shareholder has died — that is transmission, and it needs entitlement documents rather than a transfer deed.
You are only converting physical certificates to demat, which does not change ownership.
The holding is already in demat form.
